Сердечно-сосудистые заболевания (ССЗ) сохраняют лидирующие позиции в структуре инвалидизации и смертности населения. Ключевым фактором риска развития ССЗ является дислипидемия, наиболее распространенной наследственной формой которой является семейная гиперхолестеринемия (СГХС). Однако несмотря на широкую распространенность, своевременная диагностика нозологии остается крайне низкой. В некоторых пилотных исследованиях было продемонстрировано, что редким заболеванием, требующим дифференциальной диагностики с дислипидемией IIa, является дефицит лизосомной кислой липазы (ДЛКЛ). Цель. Представить результаты анализа липидного профиля 1000 детей младшего школьного возраста (9–11 лет), обследованных в рамках универсального скрининга в Санкт-Петербурге. Материалы и методы. Обследовано 1000 детей в возрасте от 9 лет до 11 лет 11 месяцев 30 дней. Дизайн исследования включал в себя оценку антропометрических показателей и уровня общего холестерина (ОХС) в капиллярной крови, в случае выявления гиперхолестеринемии оценивался семейный анамнез, биохимический анализ крови, результаты дуплексного сканирования сосудов шеи, исключались вторичные причины дислипидемии. По показаниям выполнялись анализ на активность ЛКЛ, генетическое обследование. Результаты. Выборка представлена 44,5 % девочек и 55,5% мальчиков. Среднийуровень ОХС капиллярной крови в исследуемой группе находился в пределах 4,04 ± 0,94 ммоль/л (2,43–10,9). Из 1000 детей была выделена когорта 87 человек (8,7 %) со значениями ОХС &gt; 5,2 ммоль/л: 50 мальчиков (57,5 %), 37 девочек (42,5 %), медиана ОХС 5,61 ± 1,02 ммоль/л (5,2–10,9). В результате стратификации по критериям СГХС Simon Broome у 8 детей с уровнем ОХС 7,95 ± 1,35 ммоль/л, ЛНП 6,11 ± 0,71 ммоль/л диагностирована СГХС (0,8 %), при генетическом тестировании патогенные и вероятно-патогенные мутации в гене LDLR подтверждены у 4 детей. У 8/8 (100 %) детей исключен ДЛКЛ. Заключение. В статье наглядно демонстрируется необходимость проведения универсального скрининга и оценки липидного профиля у пациентов педиатрической популяции.</p></abstract><trans-abstract xml:lang="en"><p>Background. Сardiovascular diseases (CVDs) remain the leading cause of disability and mortality worldwide. Dyslipidemia is a major risk factor for CVD, with familial hypercholesterolemia (FH) being the most common hereditary form. Despite its high prevalence, timely identification of FH remains remarkably low. Some pilot studies have shown that lysosomal acid lipase deficiency (LAL-D), a rare disorder requiring differential diagnosis from type IIa dyslipidemia, may be underrecognized. Objective. To present the results of lipid profile analysis in 1,000 primary school-aged children (9–11 years) who underwent universal screening in Saint Petersburg. Design and methods. A total of 1,000 children aged 9 to 11 years 11 months and 30 days were examined. The study design included assessment of anthropometric parameters and total cholesterol (TC) levels in capillary blood. In cases of hypercholesterolemia, family history, comprehensive biochemical blood analysis, and carotid artery duplex scan were performed, along with exclusion of secondary causes of dyslipidemia. When indicated, lysosomal acid lipase (LAL) activity and genetic testing were conducted. Results. The cohort comprised 44.5 % girls and 55.5 % boys. The mean capillary blood TC level was 4.04 ± 0.94 mmol/L (range: 2.43–10.9 mmol/L). Among the 1,000 children, 87 (8.7 %) had TC &gt;5.2 mmol/L: 50 boys (57.5 %) and 37 girls (42.5 %), with a median TC of 5.61 ± 1.02 mmol/L (range: 5.2–10.9 mmol/L). LAL-D was excluded in all 8 children (100 %). Conclusion.
